We are not accepting new Medicaid patients at the present time.

Policy Change for Form Processing:

As of January 1, 2018 there is now a processing fee of $10.00 for all forms - with the exception of forms presented at the time of arrival for a Well Child Examination, there will be no charge. As always it is requested that you allow one week for form completion.
